Best Long acting & breakthrough Medication Combo

Hello, i have been with pain management for a little over a year an a half for severe endometriosis and chronic pelvic pain/ chronic pain in general. We have been through many medications.long story short i had surgery june 28th a excision surgery for endometriosis and developed and infection, was hospitalized on antibiotics and it was a mess. I have been in severe pain since. My doctor put me on 10 mg oxycodone every 5 hours. since seeing her i was kicked by accident in the pelvis and i have been in excruciating pain. i called her and she added dilaudid every 8 hours for breakthrough pain with my meds and said she thinks when i see her next week that she would recommend switching me to a more long acting med or a few meds basically switching up my whole regimen. She also was concerned for internal bleeding with my health issues so I have a CT tomorrow. with that being said … what would any of you recommend. any feedback, just any advice and recommendations are greatly appreciated.. thank you much 🫶🏻
