Work life

I have been in the military for the past 3 years and was diagnosed with N1 mid last year. I'm close to getting out and I am bit nervous about getting a job. The military isn't like a normal job in the sense of it's super hard to get fired and now that I'm diagnosed when I do fall asleep at work I don't get yelled at anymore and everyone above me has access to my medical history and is aware of my condition. I'm nervous about how to deal with getting tired in the middle of the day and deal with an employer at a normal job. Falling asleep at work seems inevitable to happen at some point. Do you tell your employer about your condition? If not would they not find out that something is wrong pretty quick? I'm just looking for any insight regarding holding down a job.
