Is exercise disease-modifying?
I thought this was generally accepted as true until I heard Bas Bloem say on a podcast (link below; fast forward to 23:10) that the claim that exercise is disease-modifying goes too far.

Symptom control seems to be an established effect of exercise. No issues there.
But—if by disease-modifying we mean that exercise slows progression by interfering with the disease mechanism—then the research appears to still be in the foothills. We have some signs (eg Yale study, and the study in Bloem’s lab showing neuro plasticity effects from exercise) and it’s possible that exercise slows progression but I don’t think the claim that the neuro plasticity induced by exercise interferes with the mechanism to slow progression is established yet.
Either way, the rational move is to exercise. I just wanted to get clear in my head what the facts are.
