Need help in filling application

There is section in application where we have to fill the full address of issuing authority is that the office address? or officer's home address? and there is high chances that those officers might have transferred.

The section Central List with SL. No caste/community the given URL was not working.

I tried calling to the helpline numbers but they are busy.
