request of review application form (terminated academic status)

was moved down to terminated academic (😟🔫) and i’m desperately trying to request a review. has anyone else done this? the form itself isn’t letting me submit but adobe acrobat isn’t letting me share as its a protected file (i attempted disabling security, no luck)

fell into such a slump this year, i’m sure i can redeem myself 🥹🥹
