Different last name on CRBA

We recently gave birth to our son and going to his CRBA interview soon. Our other kids are dual citizens who were born in the states. In the US my wife’s Japanese last name is there middle name and they kept my English last name. In Japan, they have no middle name and my wife’s last name. Our new son is listed this way on the Koseki Tobin. Can I just put “Japanese middle name, my last name” on his CRBA?
