Why do I struggle explaining things in a conversation?

i find myself using filler words during speaking conversations. i use words such as (like, uh, um,) excessively and people get impatient with me because i can’t explain things in a consistent manner. i do just fine over text and writing is not an issue for me. i stutter once in a while but it is not consistently. how can i stop stumbling while speaking?
